Cure Chinese Medicine Clinic is a private Chinese medicine clinic in Hong Kong (hereinafter referred to as "the Clinic"). Staff at the Clinic may request you to provide personal information, which will be used for responding to feedback, registering to receive the latest news, making diagnoses, reviewing medical records, or for other purposes for which you are invited to provide personal information.

 

When you provide your personal information, please ensure that the information is accurate and complete. Failure to provide the required information, or providing inaccurate or incomplete information, will affect our consideration of your case.

 

Please note that your personal information may be provided to the following appropriate persons:

  • Anyone at our clinic who requires your personal information should do so for the following purposes: updates, diagnoses, medical record checks, or other purposes for which you have been invited to provide your personal information; or

  • If a complaint needs to be filed with the Chinese Medicine Administration Committee, relevant information should be submitted to the Chinese Medicine Administration Committee for follow-up purposes.

  • As required by relevant laws, this clinic is required to submit relevant information to the relevant government departments/agency for the purposes of such laws.

 

In addition to the points mentioned above, this clinic will only use, disclose, or transfer your personal information in the following circumstances:

  • For the latest news, job applications, medical diagnoses, medical records, or other purposes for which you are invited to provide personal information.

  • Where permitted by law.

 

Before using your personal data for any other purpose, our clinic will seek your consent.

Patient Rights and Responsibilities

Patients’ Charter

The purpose of the Patient’s Charter is to explain both your Rights and Responsibilities when you use the services of our clinic as a patient. Knowing and understanding your rights and responsibilities will make your relationship with healthcare providers a mutually beneficial one, leading to receiving appropriate treatment arrangement and enhancing the effectiveness of healthcare process.

 

Patients’ Rights

 

Right to Medical Treatment

  1. The right to receive medical advice and treatment which fully meets the currently accepted standards of care and quality.

  2. The currently accepted standards are those adopted by a responsible body of the profession in the light of accepted contemporary medical practice.

 

Right to Information

  1. The right to information about what healthcare services are available; and what charges are involved.

  2. The right to be given a clear description of your medical condition, with diagnosis, prognosis (i.e. and opinion as to the likely future course of any illness), and of the treatment proposed including common complications, risks and appropriate alternatives.

  3. The right to know the names of any medications to be prescribed and its normal actions and potential side-effects given your condition.

  4. The right of access to medical information which relates to your condition and treatment.

  5. The right to ask your medical practitioner about the fees for all interventions / procedures that are recommended.

Right to Choices

  1. The right to accept or refuse any medication, investigation or treatment, and to be informed of the likely consequences of doing so.

  2. The right to have a second medical

  3. The right to choose the types of therapy or treatment plan. Appropriate referral shall be obtained if necessary.

  4. The right to accept or refuse to participate in clinical teaching or clinical research

 

Right to Privacy / Protection

  1. The right to have your privacy, dignity and spiritual care and cultural beliefs respected.

  2. The right to have information relating to your medical condition kept confidential.

  3. The right to be protected from physical assault.

  4. Vulnerable children, disabled individuals and the elderly have the right to be protected.

Right to Complaint

  1. The right to make a complaint through channels provided for this purpose by our clinic, and to have any complaint dealt with promptly and fairly.

 

Patients’ Responsibilities

  1. Provide as much information as you can to your healthcare providers about your present health, past illnesses, any allergies and any other relevant details.

  2. Inform your medical practitioner about all the medications that you take including alternative medications (pills, medicinal herbs, tea, )

  3. Follow the prescribed and agreed treatment plan and comply with the instructions given.

  4. Show consideration for the rights of other patients and healthcare providers, by following the hospital rules concerning patient conduct.

  5. Keep any appointments that you make or notify the hospital or clinic as early as possible if you are unable to do so.

  6. Should not ask healthcare providers to provide untrue information, to issue false receipts, certificates or medical reports.

  7. Should not waste medical resources unnecessarily.

  8. Be responsible to settle the required fees and charges for the hospital services provided to you.

  9. Be responsible for the personal belongings that you bring to the hospital.
