Application Instructions for Yu Yuan Tang Free Clinic Card

To ensure that the free medical consultation and medicine delivery services benefit those in need more effectively and fairly, Cure Chinese Medicine Clinic has implemented a "free consultation certificate" system. Eligible citizens can apply in advance and participate in regular free consultations with their certificates.

Eligibility requirements (meeting any one of the following is sufficient):

1. Elderly: Hong Kong residents aged 65 or above.

2. Social security recipients: Currently receiving Comprehensive Social Security Assistance (CSSA) or High-Level Senior Living Allowance.

3. Individuals facing financial difficulties: Those referred in writing by social workers, registered charities, or community centers, or whose family income is below the poverty line set by the government (please provide proof of income for the most recent three months).

4. Cases of special difficulties: such as sudden serious illness, unemployment, or unexpected events leading to financial difficulties, which will be considered at the discretion of this council after assessment.

Application steps:

Step 1: Prepare documents

Please prepare the original and copies of the following documents:

Hong Kong Identity Card

- Proof of address for the last three months

- Qualification documents (such as: Comprehensive Social Security Assistance certificate, Social Welfare Department subsidy confirmation letter, income certificate, social worker referral letter, etc.)

Step Two: Handling in person

- Please visit the Yu Yuan Tang reception desk during clinic hours to obtain and fill out the "Free Clinic Certificate Application Form".

Submit the application form along with the above documents to the staff for verification. The originals will be returned immediately after verification.

Step 3: Qualification Review

- Our clinic will complete the review within five business days and notify you of the application result via SMS.

- If the information is incomplete or needs to be supplemented, our staff will contact the applicant.

Step 4: Obtain the free clinic certificate

- After the application is approved, the applicant must personally go to the clinic to collect the "Yu Yuan Tang Free Clinic Certificate".

- This certificate is valid for one year. If you still need to participate in free clinic services after the expiration date, you can bring the updated information to apply for renewal.

Important Reminder:

- The free clinic card is for the exclusive use of the cardholder and may not be lent to others.

- Certificate holders can use their certificates to make appointments and participate in monthly free clinic days, and enjoy "free medical treatment and medicine delivery" services.

- If lost, please notify the clinic immediately to report the loss and apply for a replacement.

- This clinic reserves the right to review applications and make the final decision on terminating service qualifications.
